Silexan is a proprietary essential oil produced from the flowers of Lavandula angustifolia (lavender) through steam distillation. It has been authorised as a medicinal product in more than 20 countries worldwide and is available in soft gelatin capsules containing 80 mg of the active substance for once-daily oral intake. Unlike lavender used in aromatherapy, Silexan is specifically manufactured and standardised for oral administration in the treatment of anxiety disorders.

The pharmacological mechanism of Silexan involves moderate inhibition of voltage-dependent calcium channels (VDCC), showing some similarities to the anxiolytic drug pregabalin. While pregabalin primarily inhibits P/Q-type channels, Silexan mainly inhibits T-type and N-type channels. For its antidepressant properties, Silexan improves several aspects of neuroplasticity through activation of the transcription factor CREB via intracellular signalling kinases like PKA and MAPK, representing a common pathway shared with antidepressant drugs. Unlike conventional treatments, Silexan is free of hypnotic or sedative side effects and appears devoid of any abuse potential.

Clinical trials have demonstrated that Silexan taken orally at 80 mg daily for 10 weeks is significantly superior to placebo in reducing both psychic and somatic symptoms of anxiety. It proved as effective as 0.5 mg/day lorazepam (a benzodiazepine) and 20 mg/day paroxetine (an SSRI) in anxiety disorders. In patients with mild to moderate major depression, Silexan was superior to placebo and comparably effective to 50 mg/day sertraline. A real-world data analysis of 67,340 adults found that long-term use (over 3 months) was well-tolerated, with rare adverse events limited to eructation (lavender burping) and mild, transient gastrointestinal complaints. No associations with drug interactions, sedation, sleep disturbance, sexual dysfunction, weight gain, or withdrawal symptoms were observed.

Silexan represents a safe and effective herbal alternative for treating subthreshold and syndromal anxiety disorders as well as mild to moderate depression, with an efficacy profile comparable to standard pharmaceuticals like lorazepam, paroxetine, and sertraline, yet without the burden of sedative effects, dependence potential, or common antidepressant side effects, making it particularly suitable for patients seeking natural treatment options requiring extended relief from their symptoms.
